Refractory hypotension in preterm infants with adrenocortical insuYciency
نویسندگان
چکیده
Five preterm, very low birthweight infants with severe hypotension and adrenocortical insuYciency are described. The profound hypotension was resistant to volume expansion and inotrope treatment, but responded promptly to corticosteroid treatment. A human corticotrophin releasing hormone (hCRH) test performed before corticosteroid treatment showed adequate pituitary response, and the endocrine dysfunction was identified at the adrenal level. Corticosteroid treatment should be considered and could be life saving in severely hypotensive preterm infants who do not respond to conventional treatment with volume expanders and inotropes. متن کاملAdrenocortical Function in Preterm Infants
Adrenal hormones are essential for the timely differentiation and maturation of fetal organs and the regulation of intrauterine homeostasis. These hormones play com plex roles during fetal life, and they are believed to provide the cellular communi cation that coordinates maternalfetal interactions. Cortisol serves to modulate functional adaptations for extrauterine life in the perinatal per...
متن کاملAdrenal insufficiency in the preterm infant
Cortisol production by the human fetal adrenal cortex has been shown to be not adequate at early gestation. This is due to an inefficient expression of 3b-hydroxysteroid dehydrogenase, the enzyme that catalyzes the synthesis of progesterone from pregnenolone, before about 23 weeks of gestation. Whereas, the fetal adrenal cortex it is able to produce dehydroepiandrosterone sulphate for placental...
متن کاملChallenges in understanding the impact of blood pressure management on cerebral oxygenation in the preterm brain
Systemic hypotension in preterm infants has been related to increased mortality, cerebrovascular lesions, and neurodevelopmental morbidity. Treatment of hypotension with inotropic medications aims at preservation of end organ perfusion and oxygen delivery, especially the brain. The common inotropic medications in preterm infants include dopamine, dobutamine, adrenaline, with adjunctive use of c...
متن کاملCortisol and 17-alpha-hydroxy-progesterone levels in infants with refractory hypotension born at 30 weeks of gestation or less.
Refractory hypotension is frequent in very low-birth weight infants, whose hypothalamic-pituitary-adrenal axis has been suggested to be immature. The objective of the present study was to evaluate basal cortisol and 17-alpha-OH-progesterone in the first 36 h of life in preterm infants with and without refractory hypotension (mean arterial blood pressure below the lower limit for gestational age...
